Summary and relationship to the curriculum: This position lends support to special needs students and their families so they will be successful in the transition to post-secondary options in accordance with the system’s mission.

 

Essential Duties and Responsibilities:

  1. Visits all special education classes/programs to observe instruction and lend support to teachers in developing an appropriate transitional
  1. Identifies outcomes and instructional options specific to the community and the individual.
  1. Develops and implements systematic program improvements in transitional programming at the preschool, elementary, middle, secondary, and post-secondary
  1. Arranges and evaluates instructional activities and materials in relation to transitional planning.
  1. Encourages the inclusion of transition-related goals in the educational program plan
  1. Develops post-school goals and objectives using interests and preferences of the individual.
  1. Matches skills and interests of the individuals to skills and demands required by vocational and post-school
  1. Interprets results of career and vocational assessments for individuals, families, and professionals. 
  1. Uses a variety of formal and informal career, transition, and vocational assessment procedures.
  1. Evaluates and modifies transition goals on an ongoing basis.
  1. Assesses and develops natural support systems to facilitate transition to post-school environments.
  1. Participates in activities of professional organizations in the field of
  1. Designs and uses procedures to evaluate and improve transition education and services in collaboration with team members.
  1. Provides information to families about transition education, services, support networks, and post-school options.
  1. Involves team members in establishing transition policy.
  1. Provides transition-focused technical assistance and professional development in collaboration with team members.
  1. Collaborates with transition-focused agencies.
  1. Assures individual, family, and agency participation in transition planning and implementation.
  1. Takes the lead in gathering data for the post-graduation activities federal report.
  1. Performs any other duties assigned by the Coordinator of Operations for Special Education, Director of Special Education and/or Assistant Superintendent for Student Services.

 

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   1. Education and/or Experience: Bachelor’s degree in Education. Graduate degree desirable. Five years of professional teaching in a classroom setting
  2. Certificate or License Required: Valid Georgia professional teaching certificate in the assigned teaching
  3. Language Skills: Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to
  4. Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole number, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar
  5. Reasoning Ability: Ability to apply commonsense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations. Ability to apply sound educational principles to various classroom
  6. Technical Ability: Must have considerable knowledge of Microsoft Office
  7. Customer Service Skills: Must be capable of interacting effectively with administrators, teachers, staff members, students, parents, and the general public using tactful and helpful customer

 

Professional Requirements: Must serve as a role model to students. Desirable personal characteristics are required. Appropriate dress and grooming consistent with established standards required. Must be at work on time. Must meet attendance standards. Must conduct activities safely.
